All patients must review Preparing For Your Visit at least 1 week prior to every appointment.
Please arrive 15 minutes prior to your appointment time with completed paperwork and other required documentation, including a current insurance card. Incomplete documents at the time of your appointment may result in rescheduling of the appointment and a cancellation fee.
TO AVOID A CANCELLATION FEE, PLEASE PROVIDE AT LEAST 24 HOURS' NOTICE WHEN CANCELLING OR RESCHEDULING YOUR APPOINTMENT. YOU MAY CALL THE OFFICE, USE THE CONFIRMATION TEXT RECEIVED ONE DAY PRIOR TO THE APPOINTMENT OR USE THE CANCEL APPOINTMENT LINK IN THE UPPER RIGHT CORNER OF OUR WEBSITE.
IMPORTANT: Having an appointment does not automatically guarantee you will have allergy testing. At the time of your consultation, one of our physicians will determine whether testing is safe and appropriate for your particular situation.
Insurance/Managed Care Participation
We accept the following insurance carriers, but not every plan from every insurer, so be sure to contact your plan's Member Services to find out if your visit with us will be covered:
- Cigna/Great West
- Coventry/Health America (recently bought by Aetna)
- Highmark/Keystone Blue/Freedom Blue/Keystone Health Plan West/Security Blue/PPO Blue
- Medical Assistance/Department of Public Welfare*
- UnitedHealthcare/Unison Community Plan/Three Rivers/MedPlus
*For established patients only
If you don't see your plan listed, please contact our office. The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff.
Review our office policies regarding prescription refills, immunotherapy injections, and Xolair injections.
